INSURANCE COVERAGE

For those with dental insurance, it is your responsibility at the time of your appointment to have your carrier's information and their exemptions. Due to the Personal Information Privacy Act (PIPA), many insurance companies will not allow dental offices to contact them inquiring about plan information. We do our best with the information you provide us.The amount set by your insurance company may be affected by such factors such as annual limits of coverage or treatment, reduced coverage for certain procedures, and/or the ability to deny coverage at any time they choose. Coverage by your insurance carrier may also vary depending on the procedure. We submit forms for insurance claims on your behalf, and as a courtesy to our patients, we will accept direct payment from your insurance carrier; however, if your insurance is negligent in paying the account, we will send you a statement for the outstanding amount. We accept the right to approve or deny your insurance. Ultimately, you are responsible for your insurance information and any charges on your account.

Co-payments, co-insurances, and deductibles are due at the time of your appointment, unless prior arrangements have been made. If arrangements need to be made depending on your insurance coverage, or financial situation, please talk to the front desk reception staff. We offer various options for payment method including cash, cheque, credit card, post dated cheques, post dated credit card, or Care Credit. Any stagnant account is subject to a collections notice, and an eventual sending to Equifax. We are very reasonable and understanding to your situation.

UNDERGRAD STUDENTS (SunLife)

Your SunLife Coverage is accepted at Campus Dental Centre. The plan is effective September 1 to August 31 the following year. If you are a new student, you may be subject to a six week black out period of eligible coverage. The plan covers 70% for basic services, such as: cleanings, xrays, fillings, extractions. This leaves 30% as the patient’s responsibility, due at the time of service. The plan has a financial maximum of $850 for the whole year. If treatment exceeds the maximum, the patient is then responsible for 100% of the amount.

GRADUATE STUDENTS (Pacific Blue Cross)

Your Pacific Blue Cross is accepted at the Campus Dental office. The plan is effective from January 1st to December 31st the same year. If you are a new student, you may be subject to a six week black out period of eligible coverage. The plan covers 70% for basic services, such as cleanings, xrays, fillings (at a reduced alternative rate set forth from the insurance company), and extractions. This leaves 30% as the patient’s responsibility, due at the time of service. The plan has a financial maximum of $750 for the whole year. If treatment exceeds the maximum, the patient is then responsible for 100% of the amount.

You may be covered under your parent’s insurance as well. If so, all information must be provided prior to your appointment.
